Home Health RN - Red Wing

Accra is a non-profit organization offering the best in personalized, self-directed home care. Accra provides employee, client, and billing services for various programs including Personal Care Assistance (PCA) Choice, Fiscal Support Entity (FSE) and Home Health Agencies. We are located in Hopkins and offer services in all of Minnesota’s 87 counties, with offices in Hopkins, Alexandria, Red Wing, Fergus Falls, Moorhead, Lake City, Duluth, Grand Rapids, Bemidji, and Brainerd. Accra is seeking a Home Health RN for its Red Wing location

General Description:

Provides Services to clients in accordance with the State Nurse Practice Act, agency policy, and accepted professional standards of practice. The Registered Nurse provides nursing services utilizing a comprehensive base of nursing theory and nursing process and communicates/documents observations and assessments. Reports to RN Nurse Manager.

Qualifications

  • Graduate of an accredited school of professional nursing. BSN, PHN preferred.
  • Current license as a Registered Nurse in the state of practice.
  • Minimum of one (1) year experience in an acute care setting or equivalent experience.
  • Current driver’s license, good driving record and reliable transportation.
  • CPR certification.
  • Strong communication and interpersonal skills.

Essential Functions

  • Assumes responsibility and accountability for the practice of professional nursing in accordance with the State Nurse Practice Act and standards for home health nursing.
    • Performs comprehensive assessments of client status, including physical, psychosocial and environmental parameters.
    • Develops and/or follows an individualized Plan of Care that incorporates analysis of data and current scientific findings.
    • Provides skilled interventions aimed at achieving realistic client outcomes within a specified time period.
    • Reports changes and information necessary to modify and update the care plan to reflect progress towards goals (outcomes).
  • Consistently demonstrates competency with technical nursing skills according to personal and legal scope of practice:
    • Assessment skills as applied to the client, family/support system and environment.
    • Teaching skills according to the client’s needs and within the context of the client’s environment, value system and physical/emotional status.
    • Medication administrations.
  • Practices nursing with respect for individual, cultural and spiritual differences.
  • Communicates effectively and in accordance with the agency requirements.
    • Reports significant changes in client status to the physician and other members of the team in a timely manner consistent with client.
    • Maintains client records showing systematic assessment, planning intervention and evaluation.
  • Initiates actions to improve the quality of patient care. Takes appropriate actions independently and responsively in-home care situations.
  • Maintains confidentiality in all aspects of the job.
  • Promotes personal safety and a safe environment for clients

Home Health LPN - Red Wing

Accra is seeking a Home Health LPN for its Red Wing location.

General Description
The Home Health LPN will perform skilled nursing functions in accordance with the federal, state, and local laws and within the guidelines of his/her professional organization and agency. All functions shall be performed in accordance with the established policies and practices and state Nurse Practice Act. The LPN delivers care to clients as delegated by the RN Nurse Manager.

Qualifications

  • Bachelor’s degree in nursing preferred, accompanied by nursing experience working in a hospital or home-health care setting. Experience with both adult and pediatric care a plus.
  • Current license as a Licensed Practical Nurse in the state of Minnesota.
  • CPR Certified
  • Demonstrated knowledge of services needed by people with disabilities and the elderly in Minnesota
  • Knowledge of federal and state statutes applicable to services for people with disabilities and the elderly in Minnesota
  • Must pass a background study with the Minnesota Department of Human Services. Have a valid driver’s license, clean driving record and reliable transportation to use for work driving purposes.
  • Demonstrated knowledge of home health care requirements, regulations and protocols a plus.
  • Effective written and Verbal Communication skills

Onboarding Specialist (Travel Required)

Accra is a non-profit organization offering the best in personalized, self-directed home care. Accra provides employee, client, and billing services for various programs including Personal Care Assistance (PCA) Choice, Fiscal Support Entity (FSE) and Home Health Agencies. We are located in Hopkins and offer services in all of Minnesota’s 87 counties, with offices in Hopkins, Alexandria, Red Wing, Fergus Falls, Moorhead, Winona, Lake City, Duluth, Grand Rapids, Bemidji, and Brainerd. Accra is seeking an individual to represent the PCA Choice program services in our Hopkins location.

Essential Functions
Duties would include but not limited to:

  • Promote Accra’s Stat Start Service by assisting customers and applicants with the application process, conducting background checks and capturing fingerprints in applicant’s homes.
  • Submit application and employee documentation to Human Resources in a timely manner.
  • Answer application and Human Resource related questions for customers in person or via phone and email providing excellent customer service.
  • Work collaboratively with (office) HR Department on application status’s while in the field
  • (In Office) Complete the NetStudy 2.0 Fingerprinting process, and support HR department as needed

Qualifications

  • Looking for personality! Must be able to adapt to all environments
  • Must be willing to drive year-round in the Metro Twin Cities
  • 1 year experience working with individuals having developmental disabilities; 2 year degree preferred but not required
  • Experience working with Waivered Services and/or PCA is a plus
  • Maintain excellent communication with families, applicants and HR Department
  • Advanced organizational skills
  • Maintain a high standard of professional courtesy, flexibility, confidentiality while working in a fast paced environment, independently and as a team member
  • Advanced skills in the use of office technology, including MS Office Suite with advanced skills in and Outlook
  • Valid driver’s license and must pass a background check

Home Health Aide in Red Wing

Provides personal care services under the direction of the RN Case Manager or Therapist. The Home Health Aide is assigned to specific clients by the RN Case Manager or other appropriate professional and performs services for clients as necessary to maintain their personal comfort. Reports to RN Case Manager.

Essential Functions
Duties would include but not limited to:

  • Performs simple procedures as an extension of therapy services under the direction and supervision of the therapist.
  • Range of motion exercises.
  • Assistance in ambulation or exercises.
  • Performs personal care activities, including but not limited to:
  • Bathing
    • Shampooing
    • Skin care/nail care
    • Oral hygiene
    • Shaving
    • Dressing
  • Performs household services essential to health care at home, including but not limited to:
  • Meal preparation/feeding
    • Laundry
    • Light housekeeping
  • Assists in the administration of medications that are ordinarily self-administered under the direction and supervision of the RN Case Manager (per state nurse practice laws and agency policy).
  • Reports any observed or reported changes in the client’s condition and/or needs to the RN Case Manager.
  • Documents cares provided and completes the forms required for the client’s records. Completes the appropriate records to document cares given and pertinent observations.
  • Promotes personal safety and a safe environment for clients by observing infection control practices, following agency guidelines, and reporting unsafe situations to the RN Case Manager.
  • Demonstrates safe practice in the use of equipment. Does not use equipment until orientation has been provided. Notifies supervisor of educational needs.
  • Communicates effectively with all members of the interdisciplinary team through verbal reports, participation in staff meetings and team conferences, as requested.
  • Maintains confidentiality in all aspects of the job.
  • Participates in required inservice programs to meet compliance requirements. Identifies learning needs to agency supervisor.
  • Performs other related duties and responsibilities as deemed necessary.

Qualifications

  • Successful completion of a formal certification training program and/or a written skills assessment test and competency evaluation.
  • Be at least eighteen (18) years of age.
  • Desired to have six (6) months’ work experience in a supervised setting, preferably health care facility.
  • Demonstrated ability to read, write and follow a written Plan of Care.
  • Good interpersonal skills.
  • Current driver’s license, good driving record and reliable transportation.
