Personal Care Assistance (PCA Choice)
Personal Care Assistance services are available for individuals, both children and adults, with disabilities, chronic diseases, behavioral diagnoses and mental illness who need assistance with personal care in their home or in the community. These services are funded through government health care programs to help individuals who are unable to fully care for themselves, and allow them to stay in their homes rather than institutional settings or hospitals.
CDCS is an unlicensed service under home and community-based waivered services. The principles of CDCS include offering the individual more choice and control over the services and supports identified through a person-centered planning process. Individuals are required to use an FSE (Fiscal Support Entity). Through consumer-directed options, individuals have increased independence, control, and responsibility over their services. Accra is a certified Fiscal Support Entity (FSE) with the state of Minnesota, offering state of the art, customer-friendly systems.
Home Health Care Services
Home care provides medical and health-related services and assistance with day-to-day activities to people in their home. it can be used to provide short-term care for people moving from a hospital or nursing home back to their home or it can also be used to provide continuing care to people with ongoing needs. Accra offers these supportive services to maintain the safety and security of the client in the community.
Customized Living Services
They are a package of personalized component services designed to meet the assessed needs of an individual living in a qualifies setting. The component services that can be included in a customized living service packages.
Respite, Personal Supports, & Homemaker
As a licensed provider of respite, personal supports, and homemaker, Accra will meet with you initially, and annually to complete a plan for services. Accra will also train your workers when they are hired and annually.
