Home Health Aide in Lake City

Provides personal care services under the direction of the RN Case Manager or Therapist. The Home Health Aide is assigned to specific clients by the RN Case Manager or other appropriate professional and performs services for clients as necessary to maintain their personal comfort. Reports to RN Case Manager.

Essential Functions
Duties would include but not limited to:

  • Performs simple procedures as an extension of therapy services under the direction and supervision of the therapist.
    • Range of motion exercises.
    • Assistance in ambulation or exercises.
  • Performs personal care activities, including but not limited to:
    • Bathing
    • Shampooing
    • Skin care/nail care
    • Oral hygiene
    • Shaving
    • Dressing
  • Performs household services essential to health care at home, including but not limited to:
    • Meal preparation/feeding
    • Laundry
    • Light housekeeping
  • Assists in the administration of medications that are ordinarily self-administered under the direction and supervision of the RN Case Manager (per state nurse practice laws and agency policy).
  • Reports any observed or reported changes in the client’s condition and/or needs to the RN Case Manager.
  • Documents cares provided and completes the forms required for the client’s records. Completes the appropriate records to document cares given and pertinent observations.
  • Promotes personal safety and a safe environment for clients by observing infection control practices, following agency guidelines, and reporting unsafe situations to the RN Case Manager.
  • Demonstrates safe practice in the use of equipment. Does not use equipment until orientation has been provided. Notifies supervisor of educational needs.
  • Communicates effectively with all members of the interdisciplinary team through verbal reports, participation in staff meetings and team conferences, as requested.
  • Maintains confidentiality in all aspects of the job.
  • Participates in required inservice programs to meet compliance requirements. Identifies learning needs to agency supervisor.
  • Performs other related duties and responsibilities as deemed necessary.

Qualifications

  • Successful completion of a formal certification training program and/or a written skills assessment test and competency evaluation.
  • Be at least eighteen (18) years of age.
  • Desired to have six (6) months’ work experience in a supervised setting, preferably health care facility.
  • Demonstrated ability to read, write and follow a written Plan of Care.
  • Good interpersonal skills.
  • Current driver’s license, good driving record and reliable transportation.

RN Case Manager- Qualified Professional

Our mission is to provide individualized services and supports to people with disabilities and older adults living at home and in their community. Accra Care provides Personal Care Assistance (PCA) Choice and Home Health services, and is also a Fiscal Support Entity (FSE). Accra Care is seeking a Registered Nurse to join our team of compassionate professionals to coordinate PCA Choice services for clients in the metro area.

Essential Functions
Duties would include but not limited to:

  • Lead intake and required home visit meetings with participants, families and PCAs
  • Educate participants and PCAs on the PCA program, rules and regulations
  • High level of comfort in a variety of home care settings and high level of cultural awareness
  • Create Care Plans for individuals based on county assessment and train staff as needed on complex medical procedures- seizure protocols, wound care, etc.
  • Communicate with Counties and participants/families on a daily basis
  • Assist in training Personal Care Assistants (PCAs) as needed
  • Responsibly maintain required program documentation
  • Assist with PCA Choice supervision as requested by DHS regulations and the participant/family
  • Maintain excellent communication with participants, families, counties and state agencies
  • Comfort with frequent driving throughout the metro area
  • Maintain a high standard of professional courtesy, flexibility, confidentiality while working in an office environment, independently and as a team member

Qualifications

  • Must have a 2 or a 4 year nursing degree, RN, and at least one year of experience with in-home services/home and community based services.
  • Experience working with Waivered Services and/or PCA and Case Management is a plus
  • Advanced communication skills both written and oral
  • Bilingual in Spanish is a plus
  • Demonstrated ability to work effectively with diverse populations
  • Ability to work effectively in a demanding, and often fluid environment
  • Ability to organize and prioritize
  • Ability to handle numerous tasks simultaneously while maintaining strong attention to detail
  • Skills in the use of office technology, including Microsoft Office
  • Valid driver’s license and must pass a background check
