Home Health RN Case Manager - Red Wing

Provides Services to clients in accordance with the State Nurse Practice Act, agency policy, and accepted professional standards of practice. The Registered Nurse provides nursing services utilizing a comprehensive base of nursing theory and nursing process and communicates/documents observations and assessments. Reports to RN Nurse Manager.

Essential Functions
Duties would include but not limited to:

  • Assumes responsibility and accountability for the practice of professional nursing in accordance with the State Nurse Practice Act and standards for home health nursing.

    A. Performs comprehensive assessments of client status, including physical, psychosocial and environmental parameters.
    B. Develops and/or follows an individualized Plan of Care that incorporates analysis of data and current scientific findings.
    C. Provides skilled interventions aimed at achieving realistic client outcomes within a specified time period.
    D. Reports changes and information necessary to modify and update the care plan to reflect progress towards goals (outcomes).

  • Consistently demonstrates competency with technical nursing skills according to personal and legal scope of practice:

    A. Assessment skills as applied to the client, family/support system and environment.
    B. Teaching skills according to the client’s needs and within the context of the client’s environment, value system and physical/emotional status.
    C. Medication administrations.

  • Practices nursing with respect for individual, cultural and spiritual differences.
  • Communicates effectively and in accordance with the agency requirements.
    A. Reports significant changes in client status to the physician and other members of the team in a timely manner consistent with client.
    B. Maintains client records showing systematic assessment, planning intervention and evaluation.
    C. Documents legibly and according to agency documentation guidelines and standards.
  • Initiates actions to improve the quality of patient care. Takes appropriate actions independently and responsively in home care situations. Initiates referrals as needed to other health team members when delegated by the RN Nurse Manager.
  • Maintains confidentiality in all aspects of the job.
  • Promotes personal safety and a safe environment for clients and coworkers.

    A. Demonstrates knowledge of safety/infection control practices by compliance with established policies and procedures.

Qualifications

  • Graduate of an accredited school of professional nursing. BSN, PHN preferred.
  • Current license as a Registered Nurse in the state of practice.
  • Minimum of one (1) year experience in an acute care setting or equivalent experience.
  • Current driver’s license, good driving record and reliable transportation.
  • CPR certification.
  • Strong communication and interpersonal skills.

FMS Financial Coordinator

Accra is a 501.c.3 non-profit home health care organization. Service Management Group (SMG) is the administrative company of Accra which provides administrative, payroll and related services to an employee group of more than 4500 individuals including a Fiscal Management Support (FMS) Agency. A FMS agency provides fiscal support to individual and families who qualify for Medical Assistance through the State of MN. Fiscal support includes budgeting, reviewing and approving timecards and expenses, and filing the necessary paperwork with the IRS or State of MN.

Summary
This position will support the Fiscal Management Services team by processing and entering payroll, expenses, W4s, and other financial records as necessary.

Essential Functions
Duties would include but not limited to:

  • Process Bi-weekly payroll
  • Enter client expenses
  • Enter employee pay rates and applicable F/EA tax exemptions, and process W-4s
  • Prepare and send out monthly utilization letters to clients
  • Process PTO payouts and retro wages
  • Set up employees with Union accounts if necessary

Qualifications

  • 1 year of payroll experience
  • Experience working with Waivered Services and/or knowledge of CDCS and CSG programs is a plus
  • Exposure to Home Health or Human Services is a plus
  • Must pass a background study with the Minnesota Department of Human Services. Have a valid driver’s license, clean driving record and reliable transportation to use for work driving purposes. FMS Payroll/Accounts Payable Lead

Program Director - Metro

The Program Director will supervise the delivery of home care services and the assigned personnel in the Minneapolis/St. Paul Metropolitan area. This position will be responsible for directing and overseeing the delivery of care under Accra licensures and monitoring outcomes for skilled and non-skilled services in the clients’ homes. This includes but is not limited to: RN/Case Managers, LPNs, Home Health Aides, Support staff and personal care assistants (PCAs).

This position will also be responsible for coordinating the utilization of outside services such as pharmacies, medical laboratories, and durable medical equipment suppliers to meet client needs. In addition, demonstrate fiscal accountability by monitoring operations to ensure compliance with Accra Care, Inc. policies and procedures, state licensure and regulations, Medicaid and Medicare standards, as well as state reimbursement regulations under MDH & DHS.

Essential Functions
Duties would include but not limited to:

  • Plan, coordinate, deliver, and oversee the delivery and outcomes of required healthcare-related services and training programs on a routine basis in cooperation with Accra Management; monitoring both skilled and non-skilled home care services.
  • Provide or Coordinate clinical teaching and conduct performance monitoring, identify and reduce risks.
  • Participate in Leadership meetings.
  • Facilitate communication and serve as a liaison between and among professional and direct care staff, family and/or guardians, and community medical providers and professional consultants in cooperation with Accra management.
  • Directly coordinate the implementation, coordination, maintenance, evaluation, and termination of care plan and services, analyses outcomes, OASIS and ICD9/10, medical records, and physician orders.
  • Follows agency process to assure clinical service audits or other quality assurance process to ensure the agency’s ability to meet or exceed local, state, and federal clinical regulatory requirements.
  • Chair committees as appropriate, e.g., safety, infection control, and incident review committees.
  • Serve as a role model for agency personnel with respect to demonstrating active listening skills and good professional boundaries in all client and vendor interactions.

Payroll Tax Accountant - Hopkins

Accra is a 501.c.3 non-profit home health care organization. Service Management Group (SMG) is the administrative company of Accra which provides administrative, payroll and related services to an employee group of more than 4500 individuals including a Fiscal Management Support (FMS) Agency. A FMS agency provides fiscal support to individual and families who qualify for Medical Assistance through the State of MN. Fiscal support includes budgeting, reviewing and approving timecards and expenses, and filing the necessary paperwork with the IRS or State of MN.

Essential Functions
Duties would include but not limited to:

  • Monitor, manage and reconcile payroll taxes at the employer and employee level. Includes reconciliation to the general ledger as well as to ADP Tax Service reports, preparation, and ensuring compliance.
  • Part of a team responsible for preparation and processing of bi-weekly payroll for approximately 5700 employees
  • Reconcile payroll tax reporting after each pay run to ensure accurate filings by tax service provider
  • Reconcile cash activity to Payroll and ADP Tax Services and In-House Services
  • Responsible for Quarterly and Yearly Tax reporting with ADP and in-house
  • Reconcile Payroll reports to 941s and W-2s
  • Responsible for ensuring the accuracy of payroll reporting
  • Reconcile payroll taxes and wages to general ledger and make necessary entries
  • Create quarterly payroll accruals
  • Gather and report requested data to internal and external recipients

IT Specialist

Accra is a non-profit organization offering the best in personalized, self-directed home care. Accra provides employee, client, and billing services for various programs including Personal Care Assistance (PCA) Choice, Fiscal Support Entity (FSE) and Home Health Agencies. We are located in Hopkins and offer services in all of Minnesota’s 87 counties including offices in Hopkins, Alexandria, Red Wing, Fergus Falls, Moorhead, Winona, Lake City, Duluth, Grand Rapids, Bemidji, and Brainerd. Accra is seeking a IT Support Specialist to become an integral part of the Accra team.

This position focuses works on basic IT Support, assisting customers and internal employees on technical questions relating to software, troubleshooting and/or setup.

General Description:

  • Certification or degree in IT Field or relevant field
  • Proficient with Microsoft Office Applications and SQL
  • Ability to manage numerous tasks simultaneously with regular interruptions while maintaining a strong attention to detail
