IT Specialist

Accra is a non-profit organization offering the best in personalized, self-directed home care. Accra provides employee, client, and billing services for various programs including Personal Care Assistance (PCA) Choice, Fiscal Support Entity (FSE) and Home Health Agencies. We are located in Hopkins and offer services in all of Minnesota’s 87 counties including offices in Hopkins, Alexandria, Red Wing, Fergus Falls, Moorhead, Winona, Lake City, Duluth, Grand Rapids, Bemidji, and Brainerd. Accra is seeking a IT Support Specialist to become an integral part of the Accra team.

This position focuses works on basic IT Support, assisting customers and internal employees on technical questions relating to software, troubleshooting and/or setup.

General Description:

  • Certification or degree in IT Field or relevant field
  • Proficient with Microsoft Office Applications and SQL
  • Ability to manage numerous tasks simultaneously with regular interruptions while maintaining a strong attention to detail
  • Knowledge and experience in Web/Website Development a plus
  • Demonstrated ability to organize, prioritize and maintain a high standard of professional courtesy, flexibility, confidentiality and discretion while working collaboratively in a small office environment, independently and as a team member
  • Ability to work with a wide variety of people
  • Self-starter with ability to take initiative and problem solve
  • Advanced communication skills, both written and oral
  • Experience working with a home health agency is a plus

Essential Functions

Duties would include but not limited to:

  • Respond to Phone Calls from internal employees and external customers in a timely manner
  • Set up desktops and accounts for new employees, and introduce the software to new hires
  • Assist the IT Department on additional projects and tasks
  • Able to assist in general office management tasks
  • Able and willing to travel to other offices in Minnesota as needed

Payroll Specialist

Service Management Group (SMG) provides employee and billing services to Home Care Agencies including PCA Choice Agencies. PCA is a program that is designed for those individuals who have qualified for Medicaid PCA Services and have the ability and desire to recruit, hire, train and schedule their PCA, or have another person who will be responsible for these tasks.

SMG is seeking an individual to assist in their high volume fast paced payroll department.

General Description:

  • Exposure to home health or human services is a plus.*
  • Strong 10 key and high volume data entry processing is required.
  • Demonstrated knowledge of services needed by people with disabilities and older adults
  • Demonstrated creativity, critical thinking, and problem solving skills
  • Excellent organizational, problem solving, communication, and interpersonal skills
  • Demonstrated a positive attitude of cross functional team collaboration and team player, both as a leader and as a follower
  • Effective communication skills, and the demonstrated ability to communicate challenging concepts both in-person, over the phone, and in electronic communication
  • Ability to adapt to rapid change and work effectively in a fast paced environment.
  • Must possess intellectual strength and have a keen understanding of internal systems needed to support services Must pass a background study with the Minnesota Department of Human Services. Qualified Professional/ RN Case Manager for PCA Services

Are you looking for a career where you can make a difference in the lives of others? Accra Care is looking for a Registered Nurse who shares our values of High Standards, Collaboration and Passion. Our mission is to provide individualized services and supports to people with disabilities and older adults living at home and in their community. Accra Care provides Personal Care Assistance (PCA) Choice and Home Health services, and is also a Fiscal Support Entity (FSE). Accra Care is seeking a Registered Nurse to join our team of compassionate professionals to coordinate PCA Choice services for clients in the metro area.

General Description
The RN QP is responsible for setting up PCA services for clients with complex health needs as determined by their PCA assessment. The QP develops the client care plan with input from the Responsible party and assessment document. They may be asked to assist with medical trainings if needed. They also serve as a resource for the client, RP or PCA on appropriate program use and other services to help meet the health and safety needs of the client in the community. The RN QP may assist in training, supervision and evaluation of services, as well as ensuring quality assurance of the PCA services at Accra Care. They are expected to stay abreast of current policies and changes to the program, and to participate in committees and meetings to enhance their knowledge of services beneficial to their clients.

Qualifications

  • Bachelor’s degree in nursing preferred, accompanied by nursing experience working in a hospital or home-health care setting. Experience with both adult and pediatric care a plus.
  • Current license as a Registered Nurse in the state of Minnesota
  • Demonstrated knowledge of services needed by people with disabilities and the elderly in Minnesota
  • Demonstrated creativity, critical thinking, and problem-solving skills
  • Able to affect change and demonstrate positive leadership
  • Excellent organizational, problem solving, communication, and interpersonal skills
  • Knowledge of federal and state statutes applicable to services for people with disabilities and the elderly in Minnesota

Activities Director (Lake City)

Accra is seeking a Part-time Activities Director for our Lake City office.

General Description
Development and coordination of activities program designed to promote the residents’ active involvement with each other, their families and the community. Providing an ongoing program of activities designed to meet the interests and the physical, mental, and psychosocial well-being of each resident. Create a monthly activity calendar, arranging outside entertainment, assessing resident preferences, and guiding the program to meet the resident’s needs.

Essential Functions

  • Creates a fun innovative recreational program for our residents, including entertainment, site-seeing, local outings, and other activities.
  • Assesses the interests and activity needs of our residents at the time of move-in and throughout their stay with us. Orient all residents to activity program and how to access activities.
  • Stimulates residents by offering activities that promote mental, intellectual, creative, spiritual and physical well-being.
  • Coordinates and plan events that appeal to our residents.
  • Prepares and posts an engaging and creative monthly program calendar which coordinates activities designed to promote the active involvement of residents living in the Assisted Living, by the 1st of every month.
  • Takes inventory and purchase activities' supplies including recreational equipment, supplies for games, and books, so as to operate within the facility budget.
  • Demonstrate involvement with all residents, residents' families, staff, volunteers and community resources.
  • Plans individual birthday celebrations for each resident
  • Maintains records of all activities, resident participation levels and acceptance of each activity by residents.
  • Observes residents’ behavior and function and reports significant changes or concerns to appropriate staff.
  • Performs work tasks in a manner that preserves each resident’s dignity, privacy and confidentiality and dignity.
  • Utilizes outside independent contractors and community resources to enrich the variety of programs made available to residents.
  • Develops and manages the department budget.
  • Able to clearly present programing through the spoken word. Can accurately communicate, providing the necessary level of detail.
  • Ensures a positive attitude is exhibited in verbal and non-verbal communication
  • Demonstrated ability to use discretion and independent judgment. Making decisions designed to achieve expectations and goals for the residents.

  • Engages and motivates residents, by developing relationships with them to encourage their program participation. Informs residents of upcoming activities and maintains a current schedule of events on the bulletin board.

Home Health Aide in Red Wing

Provides personal care services under the direction of the RN Case Manager or Therapist. The Home Health Aide is assigned to specific clients by the RN Case Manager or other appropriate professional and performs services for clients as necessary to maintain their personal comfort. Reports to RN Case Manager.

Essential Functions
Duties would include but not limited to:

  • Performs simple procedures as an extension of therapy services under the direction and supervision of the therapist.
  • Range of motion exercises.
  • Assistance in ambulation or exercises.
  • Performs personal care activities, including but not limited to:
  • Bathing
    • Shampooing
    • Skin care/nail care
    • Oral hygiene
    • Shaving
    • Dressing
  • Performs household services essential to health care at home, including but not limited to:
  • Meal preparation/feeding
    • Laundry
    • Light housekeeping
  • Assists in the administration of medications that are ordinarily self-administered under the direction and supervision of the RN Case Manager (per state nurse practice laws and agency policy).
  • Reports any observed or reported changes in the client’s condition and/or needs to the RN Case Manager.
  • Documents cares provided and completes the forms required for the client’s records. Completes the appropriate records to document cares given and pertinent observations.
  • Promotes personal safety and a safe environment for clients by observing infection control practices, following agency guidelines, and reporting unsafe situations to the RN Case Manager.
  • Demonstrates safe practice in the use of equipment. Does not use equipment until orientation has been provided. Notifies supervisor of educational needs.
  • Communicates effectively with all members of the interdisciplinary team through verbal reports, participation in staff meetings and team conferences, as requested.
  • Maintains confidentiality in all aspects of the job.
  • Participates in required inservice programs to meet compliance requirements. Identifies learning needs to agency supervisor.
  • Performs other related duties and responsibilities as deemed necessary.

Qualifications

  • Successful completion of a formal certification training program and/or a written skills assessment test and competency evaluation.
  • Be at least eighteen (18) years of age.
  • Desired to have six (6) months’ work experience in a supervised setting, preferably health care facility.
  • Demonstrated ability to read, write and follow a written Plan of Care.
  • Good interpersonal skills.
  • Current driver’s license, good driving record and reliable transportation.
