Scheduling Specialist - Hopkins Client Services Department

Job Location: Hopkins

General Description:

  • Schedule intake, regular and annual home visits with families
  • Review Service Agreements for accuracy with start dates
  • Maintain program required documentation
  • Coordinate service delivery and accommodation with other members of the Accra team, including Qualified Professionals, HR staff and others.
  • Ability to maintain a calm and professional demeanor when problem solving with occasionally demanding customers and coworkers.
  • Ability to maintain a dependable work attendance, and engage as part of a collaborative and integrated team.

Payroll Specialist

Service Management Group (SMG) provides employee and billing services to Home Care Agencies including PCA Choice Agencies. PCA is a program that is designed for those individuals who have qualified for Medicaid PCA Services and have the ability and desire to recruit, hire, train and schedule their PCA, or have another person who will be responsible for these tasks.

SMG is seeking an individual to assist in their high volume fast paced payroll department.

General Description:

  • Exposure to home health or human services is a plus.*
  • Strong 10 key and high volume data entry processing is required.
  • Demonstrated knowledge of services needed by people with disabilities and older adults
  • Demonstrated creativity, critical thinking, and problem solving skills
  • Excellent organizational, problem solving, communication, and interpersonal skills
  • Demonstrated a positive attitude of cross functional team collaboration and team player, both as a leader and as a follower
  • Effective communication skills, and the demonstrated ability to communicate challenging concepts both in-person, over the phone, and in electronic communication
  • Ability to adapt to rapid change and work effectively in a fast paced environment.
  • Must possess intellectual strength and have a keen understanding of internal systems needed to support services Must pass a background study with the Minnesota Department of Human Services. Have a valid driver’s license, clean driving record and reliable transportation to use for work driving purposes*
  • Demonstrated proficiency in the use of Microsoft Office tools.

Essential Functions

Duties would include but not limited to:

  • Processing bi-weekly employee timecards
  • Processing miscellaneous payroll paperwork as needed

Home Health Aide in Red Wing

Provides personal care services under the direction of the RN Case Manager or Therapist. The Home Health Aide is assigned to specific clients by the RN Case Manager or other appropriate professional and performs services for clients as necessary to maintain their personal comfort. Reports to RN Case Manager.

Essential Functions
Duties would include but not limited to:

  • Performs simple procedures as an extension of therapy services under the direction and supervision of the therapist.
  • Range of motion exercises.
  • Assistance in ambulation or exercises.
  • Performs personal care activities, including but not limited to:
  • Bathing
    • Shampooing
    • Skin care/nail care
    • Oral hygiene
    • Shaving
    • Dressing
  • Performs household services essential to health care at home, including but not limited to:
  • Meal preparation/feeding
    • Laundry
    • Light housekeeping
  • Assists in the administration of medications that are ordinarily self-administered under the direction and supervision of the RN Case Manager (per state nurse practice laws and agency policy).
  • Reports any observed or reported changes in the client’s condition and/or needs to the RN Case Manager.
  • Documents cares provided and completes the forms required for the client’s records. Completes the appropriate records to document cares given and pertinent observations.
  • Promotes personal safety and a safe environment for clients by observing infection control practices, following agency guidelines, and reporting unsafe situations to the RN Case Manager.
  • Demonstrates safe practice in the use of equipment. Does not use equipment until orientation has been provided. Notifies supervisor of educational needs.
  • Communicates effectively with all members of the interdisciplinary team through verbal reports, participation in staff meetings and team conferences, as requested.
  • Maintains confidentiality in all aspects of the job.
  • Participates in required inservice programs to meet compliance requirements. Identifies learning needs to agency supervisor.
  • Performs other related duties and responsibilities as deemed necessary.

Qualifications

  • Successful completion of a formal certification training program and/or a written skills assessment test and competency evaluation.
  • Be at least eighteen (18) years of age.
  • Desired to have six (6) months’ work experience in a supervised setting, preferably health care facility.
  • Demonstrated ability to read, write and follow a written Plan of Care.
  • Good interpersonal skills.
  • Current driver’s license, good driving record and reliable transportation.
